A drowning process rarely looks the way it's shown in movies. Loud shouting, frantic waving, dramatic movement - none of that happens much in reality. The reason lies in breathing physiology: once the airway repeatedly comes into contact with water, the body's entire priority shifts to maintaining its oxygen supply. Talking, shouting, and waving all require controlled exhalation. But while drowning, every available breathing movement serves only one purpose: bringing the mouth above the water's surface for a split second. As a result, the person has neither enough time nor enough air to actively draw attention to themselves.
Francesco A. Pia described this behavior pattern back in 1974, and later observational studies confirmed it. What's known as the "Instinctive Drowning Response" isn't a conscious action - it's an involuntary, evolutionarily rooted survival response to acute oxygen deprivation, regardless of age, gender, or swimming ability. Once breathing is under threat, the body suppresses almost every other voluntary movement.
Characteristic signs: the body stays almost upright in the water, and the legs barely move in a coordinated way anymore. The arms press sideways or slightly downward - not to wave, but to briefly generate lift and raise the mouth.
Purposeful movement is usually no longer possible: people drift almost motionless or slowly sink, while often still looking as if they're calmly standing in the water. For bystanders, this is exactly the problem: without dramatic movement, the situation is easily misjudged. At busy swimming spots, drowning people are therefore often only noticed once they've already gone under - even if they were in plain sight for a while. This shows that recognizing an emergency isn't an intuitive skill - it has to be specifically trained.
The Stage Before Drowning: Aquatic Distress
Before the actual Instinctive Drowning Response, international water rescue organizations distinguish an earlier stage: aquatic distress. Here, the person is still capable of acting, but already in a critical situation - usually due to exhaustion, early cramping, disorientation, strong currents, or psychological overload. In this stage, people can often still actively call for help or make purposeful movements. This is exactly where early help - such as a flotation device - has the best chance of preventing the progression to actual drowning. Prevention and rescue, in other words, begin before someone actually goes under.

From the Exhaustion Cycle to Hypoxia
The shift from a manageable emergency to actual drowning often happens gradually. As exhaustion grows, the ability to keep the head above water decreases, while breathing rate increases from the effort. Once water repeatedly reaches the mouth and nose, the breathing rhythm is thrown off - even small amounts of inhaled water trigger coughing reflexes and disrupt breathing further. Exhaustion, psychological stress, and early hypoxia - a lack of oxygen in the body - reinforce each other: the effort of breathing increases, oxygen demand grows, and oxygen uptake keeps getting worse at the same time. This self-reinforcing cycle can significantly worsen the situation within a short time.
As hypoxia progresses, behavior changes too:
- Reduced oxygen supply to the brain first impairs attention and judgment. People react with delay, make illogical decisions, or are no longer able to carry out simple rescue actions.
- Disorientation, confusion, and eventually unconsciousness follow. This explains why drowning people are often unable to use even nearby rescue equipment - such as a life ring within reach.
This is exactly why intervening as early as possible matters, while cognitive function is still intact.
Raising the Alarm: How to Alert Help Correctly in a Drowning Emergency
Just as critical as recognizing the emergency is immediately alerting professional help. The Drowning Chain of Survival doesn't treat alerting emergency services as an administrative afterthought, but as a fixed part of medical care. If several people are present, emergency services should be called immediately, in parallel with first rescue efforts. International guidelines explicitly recommend dividing tasks early to save time: while one person prepares the rescue or provides flotation, a second person calls emergency services and gives as precise information as possible about the location, the number of people affected, water conditions, and the condition of the person in danger.

Precise location is especially important in open water. Lakes, rivers, or stretches of coastline often don't have a clear address. Modern rescue systems therefore increasingly rely on GPS coordinates, digital rescue point systems, or smartphone-based emergency apps to guide responders to the scene faster. Even small delays in finding the location can significantly worsen the neurological outcome. Alerting help, then, doesn't end with the emergency call - it also includes continuously guiding responders until they arrive.
Why Bystanders Often Don't Step In: The Bystander Effect
From a human factors perspective - the study of how people behave in dangerous situations - recognizing an emergency is a complex perceptual process. Attention is limited and prone to bias. At busy bodies of water, what's known as the bystander effect often occurs: the more people are present, the less responsible any single person feels to step in or get help themselves. At the same time, people take cues from how others around them behave. If bystanders stay calm, a real emergency is more likely to be underestimated. These psychological mechanisms are well documented in accident research and show that successful water rescue takes more than medical knowledge - it also takes the ability to recognize danger early and act decisively.
